iHealth shares the effectiveness of chronic disease management and control in the state capital

On September 26, iHealth hosted a significant event in California’s capital, celebrating the rapid growth of its Unified Care program and the opening of a new office. The occasion was led by iHealth Senior Vice President Susan Wu, alongside CEO Jack Feng, who gathered with local dignitaries and healthcare partners to discuss the impressive achievements of the Unified Care initiative in supporting patients with diabetes and hypertension. Approximately 150 guests attended the event.

While iHealth is widely recognized for its orange COVID-19 testing kits, its accomplishments in chronic disease management are equally noteworthy. The Unified Care program focuses on delivering high-quality management services to community members dealing with diabetes and hypertension, significantly enhancing their quality of life. This event was not only a celebration of the project’s success but also marked the official opening of iHealth’s new office in Sacramento and the fifth anniversary of the company’s collaboration with physicians in the area.

Headquartered in Sunnyvale, iHealth established a new regional office in 2023 in the capital region. Due to the rapid expansion of the Unified Care program in both the capital and Bay Area, the company relocated to a larger office space last month, increasing its workforce by 2.5 times. This growth reflects iHealth’s ongoing commitment to remote patient monitoring and chronic disease management, showcasing the success of its innovative care model.

The current Unified Care team includes registered dietitians, health coaches, and clinical assistants, providing support to nearly 20,000 patients with diabetes and hypertension. Through a combination of online and offline care, the team offers personalized chronic disease management services tailored to the unique needs of the aging population in the community. Since the program’s launch in 2018, the team has worked closely with physicians to bridge the gap between doctors and home monitoring for patients. Utilizing remote vital sign monitoring devices, nutritional counseling, lifestyle coaching, and continuous care coordination, they have significantly improved clinical outcomes.

Notably, the iHealth Unified Care program has achieved remarkable clinical results, with a 75% reduction in poorly controlled blood sugar levels among diabetes patients and a 45% decrease in uncontrolled cases for hypertension patients. Within just six months, the proportion of hypertension patients achieving good control increased by 78%. At the event, Jack Feng emphasized that the company maintains a 95% patient satisfaction rate, delivering the highest level of personalized support—a driving force behind the program’s continued growth.
